  • Be sure all fields are exactly like the credit union has on file.
  • If you have moved or married, be sure to use the data that the credit union has on their system.
  • Try to use the area code if that doesn't work, try without it.
  • Be sure to follow examples to the right of the form fields exactly.
  • Be sure to add the slashes between the city/state/zip.
